Depression After a Heart Attack Dangerous for Years

FRIDAY, March 7 (HealthDay News) — The increased risk of death associated with depression after a heart attack persists for at least five years, a study finds.

“We’ve known for a number of years that depression increases the risk of mortality as well as morbidity [illness] after a heart attack for at least three to six months,” said study author Robert M. Carney, a professor of psychiatry at Washington University School of Medicine in St. Louis. “We assumed that we would find a decline in risk, but that was not what we found. The risk remained worse after five years.”

Carney and his colleagues followed more than 750 people after their heart attacks, according to their report in the current online issue of the Journal of Affective Disorders. Using diagnostic interviews rather than the self-reporting common in most such studies, the researchers determined that 163 had major depression, and 195 had minor depression. Over the five-year study, the death rate was 87 percent higher for those with major depression and 76 percent higher for those with any form of depression.

In real numbers, 62 people diagnosed with depression died during the study, while 44 non-depressed heart attack patients died. Why depression should increase the risk of dying is a mystery, Carney said. “We think that because depression is a chronic and recurrent problem, the factors causing that risk recur over time,” he said. “But we don’t know the mechanism.”

The researchers have started a trial to determine whether the omega-3 fatty acids that are found in fish oil can reduce that risk. Heart patients are being given an antidepressant drug and a special formulation of omega-3 fatty acids, comparing them with a similar group that gets only an antidepressant.

“A number of studies over the years have found an inverse relationship between the amount of fish people eat and depression,” Carney said. “The advantage of giving them in heart disease is that they have an effect on the cardiovascular system as well.”

The major finding of the study and the use of omega-3 fatty acids are already in the mainstream of research on depression and heart disease, said Dr. Alexander H. Glassman, a professor of psychiatry at Columbia University Medical Center in New York City.

“There is a torrent of information that depression in relation to vascular disease worsens the outcome,” Glassman said. “If you look at post-stroke patients, you find the same data. If you look at heart failure, depression has a similar effect on mortality.”

The value of the study is that it had the longest follow-up of any trial using diagnostic interviews, which are regarded as more accurate than self-reporting, Glassman said. “It makes the evidence firmer and extends the evidence,” he explained. The use of omega-3 fatty acids is “a hot issue,” being tried in cardiac and non-cardiac cases, Glassman said. “It is a logical thing to do,” he added. The study and the omega-3 trial will still leave some major issues about depression and heart disease open, Glassman said.

“The two key questions that remain are: Does treating depression make the outcome better? And what is it about depression that is causing the problem in the first place?” he said.

Everyone feels down every now and then, and this is normal. However, clinical depression is lasting and intense - the gloom can be overwhelming to an individual. Some sufferers describe clinical depression as “living in a black hole,” or having a feeling of despair that they cannot escape from. Some individuals suffering from clinical depression don’t feel sad at all - instead, they experience a lingering feeling of emptiness. They are generally apathetic and numb towards enjoyment and pleasure, even when going through activities that they normally enjoy. This is usually a clear sign of clinical depression.

For a doctor or other health professional to diagnose clinical depression in an individual, most of the following signs and symptoms must be observed in the patient for at least two weeks. Problems with sleep. Normal individuals exhibit a fairly steady sleeping pattern. Having trouble getting to sleep (or its opposite - getting too much sleep) is typical of clinically depressed individuals. When an individual wakes up in the middle of the night, or early in the morning, and finds himself/herself unable to go back to sleep, it may be a sign of clinical depression.

Hampered thinking or concentration. A clinically depressed individual may have trouble concentrating, making decisions, and remembering. Such problems may come into conflict with a person’s performance in school or work, and may render the individual unable to hold a job until the episode passes.

Weight changes. Clinical depression may cause an individual to lose or gain weight suddenly. Abrupt appetite changes are also indicative of this. Weight changes will also lead to a host of other health problems (such as high blood pressure), further aggravating the individual’s condition.

Irritability and anxiety. Clinically depressed individuals may seem unusually restless, agitated, irritable, and easily annoyed. They may also feel fearful or anxious for no reason. For many individuals suffering from depression, this is the first symptom that the people around them notice.

Marked fatigue. Depressed individuals feel weary nearly every day, or at least notice a pronounced dip in their energy levels. Some individuals may wake up from a night’s sleep feeling just as tired as they were the night before. Others may even feel like they’re moving in slow motion, or speak in a slow, monotonous expression.

Self-esteem suffers. Clinical depression almost always causes an individual to feel worthless or guilty, often for no apparent reason. Reduced interest in sex. Individuals normally sexually active may notice a conspicuous decrease in interest with regards to having sexual relations. For most people, this is cause enough to worry.

Thinking of death. Depression may cause individuals to see themselves, their situations, and the future very negatively. This may lead to thoughts of death, dying, or even suicide. Aside from the above-noted symptoms, depression can also cause a host of physical problems, such as gastrointestinal discomfort (indigestion, diarrhea, constipation, etc.), headaches, backaches, and muscle pain. source

As we grow older, and unfortunately whether we like it or not, our metabolisms change and so with it change our requirements for different medication and a different dose. And often as one may be diagnosed with depression once, it may never go away to the point that the patient can live his/her live without medication completely. Adjustments may become necessary, just when you thought you have finally found the medication.

Our philosophy and suggestion is to trying to find a natural method to trying to improve the wellbeing of humans at the workplace, not as a replacement to the required medication, but as a supplement to enhance and improve the already depressed lifestyles we often lead. This is ideal for those with diagnosed depression. But those who think they are not depressed, they too may benefit from fresher cleaner pure air inside their home and at the work place. They also may want to benefit from freshly prepared healthy fruit and vegetable smoothies boosting their morning drive in the beginning hours of their workday. And of course, exercise does nobody harm, if done appropriately.

Further, our theory is that when people feel more energetic because of better and cleaner air and because of a freshly blended orange juice and after climbing up the stairs to the seventh floor instead of taking the elevator, it could just make our society more productive.
